Potential HIPAA Breach Notification - Bridgeway Advantage D-SNP Members
Notice to Bridgeway Health Solutions Medicare Advantage (DNSP) Members
On October 8, 2015, Bridgeway Health Solutions learned that a former employee downloaded data files containing member protected health information (PHI) to a personal device. This happened between March 8, 2015 and March 17, 2015. The employee left the company on March 18, 2015. The former employee did not have permission to download the files. His actions were against company policies.
On October 25, 2015, Bridgeway contacted federal law enforcement who began an investigation. Law enforcement ended its investigation on April 5, 2016 and did not find the storage device. They did not find any evidence that the member information has been used wrongly.
Bridgeway takes member privacy seriously. Bridgeway has reviewed its security rules and has updated equipment tracking procedures to help prevent this from happening again.
Members who were affected will be offered free credit monitoring and identity theft protection services. The information that was disclosed included members’ name, Medicaid ID, Medicare ID, and member ID, and in some instances, address, date of birth, and social security number. No credit card or financial information was shared.
Bridgeway regrets any concern this may have caused. Please call Bridgeway Medicare Advantage DSNP Member Services with any questions:
