Care Coordination and Disease Management
Care coordination and disease coaching are part of your health benefits and are provided to you at no cost. Bridgeway Health Solutions pays for these services. We provide services for many conditions, such as asthma, diabetes, COPD, high-risk pregnancy and many more. Call Member Services at 1-866-475-3129 (TTY/TDD 711).
Cell Phone Program
Bridgeway Health Solutions is proud to be working with SafeLink Wireless to offer you this special, federal program. As a Member of our plan, you get all the same benefits of a SafeLink phone, plus unlimited inbound text messages and calls to Bridgeway Health Solutions member services. There is no added cost for these extras.
How to Enroll:
Choice of Doctor
As a Bridgeway Health Solutions Member, you can choose who you see for your healthcare needs from our network of Providers. We have many for you to choose from. If you need help choosing a doctor, call your Case Manager or Member Services.
Bridgeway provides all members under the age of 21 with all medically necessary dental services. If you are under the age of 21, Bridgeway will assign you to a Dental home - a dentist you see every six months for check-ups that include a cleaning and fluoride treatment. Talk to your Case Manager if you have questions about the dental services available to you as a Bridgewaymember.
Doctor Visits & Screenings
Preventative care visits are covered by Bridgeway Health Solutions. This means visiting the doctor for a regular check-up, rather than waiting until you are sick.
Bridgeway Health Solutions covers an annual physical for adults.
Regular child check-ups are also covered for those under the age of 21. These doctor visits should occur at the below ages. Children also need a blood lead test at 12 and 24 months of age. We cover this. After that, if they are high risk, they need the blood lead test each year age 3 to 6.
- Birth or neonatal exam
- 2 to 4 days for newborns discharged less than 48 hours after delivery

Nurse Advice Line
Our Nurse Advice Line is ready to answer your health questions 24 hours a day – every day of the year. It is staffed with Registered Nurses. These nurses have spent lots of time caring for people. Now they are ready and eager to help you. Call 1-866-475-3129 and say "Nurse."
Other Languages & Format Needs
Interpreter services are provided free of charge to you during any service or grievance process. This includes American Sign Language and real-time oral interpretation.
If you need something translated into a language other than English, please call Bridgeway Health Solutions. We can also provide things in other formats such as Braille, CD or large print.
If you need an interpreter for your medical appointment, contact Bridgeway Health Solutions 48 hours before your appointment. We will arrange for one to be at your appointment.
Non-Emergency Transportation Services (NET)
Bridgeway can help you get a ride to your covered healthcare visits. If you live in a nursing home or an assisted living home, their staff will arrange a ride for you, including an ambulance. If you live at home, it is important to first ask a relative, friend or neighbor if they can give you a ride.
How to Get a Ride
Call Total Transit at least three days before your appointment if you can. The number for Total Transit is 1-877-986-7420.
- Tell Total Transit if you are in a wheelchair and need a van that can load your wheelchair.
If you cannot keep your appointment, please call Total Transit to cancel your ride.

- If you have an urgent or same day need, Bridgeway will do everything they can to schedule a ride for you.

Please remember to first arrange transportation with a relative, friend or neighbor. If they cannot help you, Bridgeway will help you find a ride to your appointment.
Bridgeway Health Solutions covers ambulance transportation to the nearest hospital for emergency situations. You do not need to contact Bridgeway Health Solutions before using an ambulance for emergencies.
It’s Never Too Late to Quit Smoking!
Do you smoke and want help to quit? It can be hard to give up the habit because nicotine is addictive. Many people feel cravings and have withdrawal symptoms when they try to stop smoking.
The good news is there is help to kick the smoking habit.
- Tell your doctor that you want to quit. Your doctor may prescribe a medicine to reduce cravings that make it hard to stop smoking.
